MEDICAL SURVEILLANCE PROGRAM - CareSpot

Steps to participate in the Occupational Health Program (all steps must be completed)

STEP 1: Fill out all required forms

  • Meet with your supervisor to complete the Initial Risk Assessment Form based on your job activities and save the form electronically to your computer.
  • If your work will require the use of a respirator, complete the 国产自拍 Respirator Medical Evaluation Questionnaire and save it electronically to your computer.听听
  • If your work will require scientific diving, go to the Diving Safety Website and contact Jimmy Nelson @ jnelso40@fau.edu.
  • If your work will require contact with research animals, complete your animal contact form in (ensure PI has added you to the lab).

STEP 2: Verification that all forms are complete

  • If you work at HBOI, meet with Tina Angelo tangelo@fau.edu to ensure all required forms have been completed and receive additional information regarding scheduling your appointment with CareSpot. You must take all forms provided to you in Step 2 to your appointment.
  • If you work at all other campuses, Contact Rich Brust at brustr@fau.edu to ensure all required forms have been completed and receive additional information regarding scheduling your appointment with CareSpot. You must take all forms provided to you in Step 2 to your appointment.

STEP 3: Schedule and attend your medical evaluation appointment

  • Contact CareSpot at your preferred location to schedule and attend your appointment. You must take all forms provided to you in Step 2 to your appointment.

STEP 4: Receive notification regarding your clearance status

  • Provider notifies worker and EH&S of clearance status following evaluation.

STEP 5: Keep your medical surveillance current

  • Renewals are required, depending on the medical surveillance program.听听
    • Animal contact annual renewals do not require in-person appointments unless recommended by the provider (on a case-by-case basis).
    • All other renewals require in-person appointments with the provider.
